本片是克林伊斯威诸多作品中极罕见的灵异题材的电影，在片中他饰演一名资深的联邦探员，他锲而不舍的精神和精准的办案手法，让他成为重案组犯罪克星，歹徒闻风丧胆。但是当他面对最新的敌人，一名被称为「密码杀手」这个近年来罕见的嗜血杀人魔时，他却不幸心脏病发作，在医生的郑重警告下，他不得已选择退休，并且动了换心手术。做了换心手术之后的泰瑞，尽管偶尔会看到一些无法解释的影像，但是笃信科学的他只当作是手术后的后遗症，然而过了不久，有一位自称是被害人的姊姊找到了泰瑞，他的妹妹惨遭连续杀人魔杀害，根据可靠的消息，凶手应该就是「密码杀手」的连续杀人魔，不只如此，这个「密码杀手」似乎早就知道泰瑞等著做换心手术，他刻意杀害这个女孩，并且让她的心脏顺利地移植到泰瑞的胸腔里，「密码杀手」一次玩弄了两个人的生命。泰瑞这才明白他为什么会看到一些无法解释的奇怪影像，为了这个救了他一命的无辜少女，他决定重出江湖，凭藉他多年来钻研连续杀人犯的心理，再加上他丰富的办案经验，相信一定可以水落石出。\r　　只不过对手连泰瑞的健康状况都了若指掌，跳动在泰瑞胸腔中的女孩心脏更是「密码杀手」精心设计的血腥邀请函，对手很明显地是冲著泰瑞而来，一切似乎都在敌人的掌握之中，泰瑞要如何面对这个以杀人为艺术的连续杀人魔？
